It seems to be a tangible piece of sickness. 20 fragments can be combined into the Pestilent Crystal which can be sold for 250 rubles. It will also unlock the Plague Incarnate achievement and the Incarnation of Gloom toy.
Can be obtained on Jigran by killing creatures with a crown symbol over their head.
Can be sold to merchants for 10 rubles.
