Introduction
Let me take a good look at you, the "Imperial survivor"...

The most probable explanation is that the portal went off because it was near you for a fairly long time. It became attuned to you! We need to confirm this hypothesis or prove it wrong.
Here is a magical artifact. It's a copy of the teleporter that pulled you out of the Astral. Carry the teleporter with you, it needs time to attune itself to you. Shouldn't take long!
For starters, go to the slums where the giant termites live. You can find a lot of them in the Old Square. After you kill four termites, try to activate the teleporter and come right back here. I hope it works... I'll be looking forward to your return.
